Retreatment Plans

At QualSight, we trust our network of credentialed LASIK surgeons to provide you with the best quality care. As a sign of this trust, we provide our Members with a one-year Provider Retreatment Plan at no additional cost.

Annual Renewable Plan – The Annual Renewable Plan will assist patients in maintaining their best possible vision with free re-treatments (except for the laser manufacturer’s licensing fee of $110-$300 per eye) when they are medically advisable within the first year of the initial procedure. To extend the Plan after the first year, simply call QualSight to schedule an exam before the anniversary of your procedure to renew your Plan for another twelve months. Provider participation may vary.

Assurance Plan – The Assurance Plan provides the Member with retreatments during the first year after initial surgery at no cost for physician services, however the Member has to pay for the laser manufacturer’s licensing fee ($110-$300 per eye). If the Member purchases the Lifetime Assurance Plan from one of our Providers the Member obtains no cost retreatments not only for the first year after surgery (including no manufacturer’s licensing fee) but also in subsequent years as long as they are medically advisable. Provider participation may vary.
